I had the same thing happen and I thought it was the bolts. However it turned out to be a very small crack in the head partially under the bracket. I believe that the area right in front of the bracket on the head is a weak (thin) area of the head. I ended up getting the head welded - lasted about an hour then I bought a new head from Moyer.
